LSA - TERMS AND CONDITIONS 2024/2025
1.Payments for the session must accompany the registration form and waiver signed; if you are unsure of the amount of payment, please contact the London Soccer Academy Office. Cash, e-transfer, credit card or make all cheque(s) payable to: The London Soccer Academy Inc. Verbal registrations cannot be accepted. The payment should be made each month on the day your child joined LSA regardless of the classes missed. An exception to this rule will be children who are once a week and therefore, their days will be accordingly adjusted.
2. A missed class for a reason such as illness must be shared with LSA - a 24 hours notification is required in writing or for unforeseen reasons you have until 9.00am the day of the session. This process is in place for organizational safety reasons. If there is no notification of the absence, your child will not be on the pickup list for that class. A maximum of 2 make-up classes will be allotted, per season, for illness. Missed classes for any other reason are not eligible for a make-up. Seasons: Fall: (Sept to Nov), Winter: (Dec to Mar), Spring: (April to June) & Summer Camp: (June to July)
3.Refund requests must be received by email to (info@londonsocceracademy.com) not more than 5 days in advance before the LSA new session begins. All refunds are subject to a $60 administration fee per refund request minus the classes attended or the credit can be applied to a future program pending availability. Regarding cancellations, making changes to the schedule or of opting out of the weekly sessions/classes, Parent need to give us (2) weeks written notice and a $60 dollars administration fee will be applicable. If the child wants to leave the soccer program completely, LSA will require (30) days written notice before the next new sessions/program commences. All requests must be received in writing. Refunds will be processed via the same method in which payment was received, with the exception of cash, in which case a cheque will be issued.
TERMS OF AGREEMENT
1 I/We agree to allow my child to participate in all child care activities during the training sessions.
2 I/We agree to release and indemnify LSA from any and all claims for damages arising as a result of accident, injury, or other wise sustained by the herein named child, arising from participation in any camp activities. Please note: shin guard and soccer shoes are mandatory.
3 I/We give permission to the staff of LSA to seek medical assistance and treatment for my child in the event of an emergency, if parent or emergency designate cannot be contacted.
4 I/We understand that the camp does not accept the responsibility for damage to, or loss of, personal belongings during the program.
5 I/We understand that the Program Director has the right to terminate the registration of any student when it is deemed by the Program Director to be in the best interests of the child, or the soccer Program.
6 I/We give LSA permission to use any photographs/videos of my child taken during soccer small sided games, practice and on the website or for camp promotions only.
7 I/We will give 15 days notice in writing prior before leaving the LSA soccer program. Medical/liability Waiver:
8 As parent/legal guardian of the named minor, I have verified that the information on this form is correct and I grant permission for this minor to participate in all activities of this sports program. I assume all risks and incidental to such participation, including risks of serious injury, and do hereby release and waive all claims against The London Soccer Academy Inc, and its staff and other participants.
9 I/We have read, and understand all the information in this registration package, including the terms of agreement, registration and refund policies, and agree to abide by the conditions outlined above.
